Gourami with a hole in its body?

My opaline gourami has this slowly growing hole in its body by the base of his left fin and possibly a tiny one growing near the top of its head. Looks brown and deep, like a large gunshot wound. At first I noticed a discoloration and thought it would go away with a little bit of aquarium salt, but it hasn’t for a while now and when I looked closer, I discovered its a hole, as if the gourami has some kind of flesh eating virus, eating the hole, making it slowly larger and deeper over time. A few months ago its body looked large, but I treated him with salt and he went back to normal, not sure if that is related. I do a 25% water change once a month. I also have a pearl and 3 spot gourami. I don’t think they have anything to do with it. What is this and how can I treat it?
My tank is 29 gallons. I see occasional chasing, but the aquarium is heavily planted with lots of hiding places.

How big is your tank? What are the water stats? You should be doing a 25% water change every week, not every month.

It sounds like a bacterial infection that has been brought upon by fighting. A lot of times gouramis fight for dominance. Do you ever witness them fighting?

In any case, separate the injured one from the others into an established tank, and treat with anti-bacterial medicated food, as well as some external treatment, such as melafix. Many people say melafix does more harm than good, but that’s really only the case for bettas. It does help a bit. Try to get an anti-bacterial med with tetracycline and treat with that instead of with melafix if you can find it.

Hole in the Head disease creates dents in the head and along the lateral lines, it DOES NOT create red lesions.

And I know this is bacterial, because my gouramis have had this before.
